This application process includes a step-by-step guide to completing your application.

It is very important you read and understand the application process and complete it accurately and truthfully.

Failure to follow the process could lock you out of the application process forever.

If you require clarification you should request assistance and clarification from the person who introduced you to the plastic waste to oil mission.

Please understand that the process and Terms & Conditions have been put in place to enable applications to be made and approved efficiently whilst simultaneously rejecting false and/or unjustified applications thus protecting the integrity of the CSR token.

You have selected Redeemed FLPs

You should choose this option if some, or all, of the ISF tokens you held at 00:01 CET on 6th March 2021 were acquired by you redeeming FLPs.

NOTE: Holding of ISF due to the prior holding of ORTP also acquired by way of redemption of FLPs is considered a valid holding of ISF for this application process.

You have selected Purchased ISF

You should choose this option if some, or all, of the ISF tokens you held at 00:01 CET on 6th March 2021 were purchased for proper value.

You have selected ISF held at Mountain Wolf

You should choose this option if some, or all, of the ISF tokens you held at 00:01 CET on 6th March 2021 were held ISF held at the Mountain Wolf exchange.

NOTE: We will contact Mountain Wolf to verify the details.

You have selected Other

You should choose this option if some, or all, of the ISF tokens you held at 00:01 CET on 6th March 2021 are not covered under the previous categories.

1. You are strongly advised to make a more specific application under the categories of "Redeemed FLP", "Purchased ISF", and "ISF held at Mountain Wolf"

2. All holders of ISF can make an application under the discretionary award process.

3. There is no guarantee that applications of type "other" will be considered as valid.

You are encouraged to make your application of category other ONLY if the other categories do not apply.

4. Each application will be reviewed separately.

By submitting this application I confirm:

1. I want to apply for a discretionary award of CSR tokens based on my holding of ISF tokens at 00:01 CET on 6th March 2021

2. I am an Active Amplivo member and I am contributing to the cause of Amplivo in an appropriate manner.

3. I understand there are many individuals who are no longer active Amplivo members and it is with regret that this application process is not suitable for such holders.

4. Only one application of this type is permitted for an Amplivo account/member.

5. If my application is rejected, I cannot raise a new application.

6. Once I submit my application, it cannot be changed in any way.

7. Successful awards of CSR tokens are made to the same EOA on the Ethereum Blockchain where the applicable ISF tokens are held.

8. Making or attempting to make a fake, false, and/or fraudulent application will lock me out from the discretionary award and may mean my Amplivo account is suspended permanently.

9. I understand it is anticipated that new applications will be processed within 28 days, often much sooner, and I will be able to review the status on the CSR portal at https://csrnow.com

10. I may appeal the rejections as outlined in the appeals process detailed on the CSR Information Portal.

11. My errors in the application process will not be considered as a reason for allowing an appeal and I will have lost the opportunity to take part in the discretionary awards process.
